Palliative Care and Outreach Services
Palliative Care and Outreach services are designed to help people locate and
obtain all the services in their community that will help them maintain their
independence, while also providing a consistent communication link to their
physician and his/her office staff in order to reduce the severity of the disease
symptoms.
The services are set up for:
- Patients in hospitals, who after discharge, could benefit from
follow-up monitoring for symptom and pain management;
- People receiving aggressive treatments for cancer which have
debilitating side effects;
- People receiving dialysis treatments and;
- Anyone who can benefit from them because of their specific
situation.
If you are eligible to receive palliative outreach services, we believe that timely
assistance will help you remain independent and that timely communications with
your physician and their office will assist them in better meeting your healthcare
needs.
Here’s How It Works
Once you are accepted into the program, you will be contacted for a structured
assessment of your individual situation. Then we'll work together with you to
develop a list of suggested services that you agree will help you continue living
independently. The services may include:

Special Diets Shuttle Service
Medical Equipment Senior Services
RX Delivery SeniorRx
Support Groups Home Health
Advance Directives Chore Workers
You will also be provided with telephone numbers to call anytime you need
assistance or have any medical questions about additional services that may be
available.
Thereafter, you will be contacted periodically, depending upon your wishes, to
check on how you are doing. Based on your feedback, we will furnish your
doctor's office with information about how you are doing health-wise and will
follow-up on any questions you may have that will help you maintain your health
status.
Obtaining Services
There is no formal process for obtaining Palliative Care and Outreach services.
You may be referred by anyone, you can call our office at 573/431-0162 or
800/876-0162, or you can fill in the information below if you believe the services
will be of benefit to you. When you reach our office, ask to speak to the
Palliative Care and Outreach Services Director to obtain additional information
or arrange for a personal visit to assess your situation.
